Records will be provided as permitted by applicable state and federal law. Fees for copies will be limited to allowable charges for duplication and delivery (for example, copying and postage). Requests that require clinical summaries, written reports, court-related documentation, or extensive record preparation may be billed at the applicable professional rate outlined in the Client Agreement.
How can a third party obtain my records?
If you would like your records released to another person or organization, you will need to sign a written authorization (release of information) that includes the recipient’s name and delivery information.Records will be released only as permitted by applicable state and federal law.
